Gain peace of mind knowing all your wishes are in writing by attending our free advanced directive event hosted by Care Initiatives Hospice and Elier Place Senior Living.
Members of our team will be on-site to assist you with completing an advanced directive or power of attorney form. We will have blank advanced directives to complete and a notary available. Be sure to have a current photo ID with you for our notary.
